Broker Consent Form - FAQs
Our digital consent form was built for efficient consent capture. The details of the form are directly based off of the form our founder developed for NABIP. Here is the link to the physical form, and it is free for NABIP members to print and use as a static document. - https://nabip.org/membership-resources/compliance-corner/individual-market-ffm-consent-form Alternatively, if you are a member of HAFA, you can access the static form that the HAFA team put together that helps as well! You can find the document in the member resource library here - http://www.hafamerica.org/resource-library/
The good news is CMS has allowed consent to be given for the entire household by one individual. To provide household consent, the person obtaining insurance should be an authorized member of the household. This individual can be anyone who has the legal authority to make decisions on behalf of the household, such as the head of the household or a designated representative.
No, any unused form submissions will not carry over from month to month, the number is just a cap. We based the solution knowing that the majority of the submissions will be during Open Enrollment.
For GAs/IMOs/FMOs we have the system set up as a pooled subscription at $0.02 per submission in packs of 10,000 submissions per month. That $0.02 is all we currently need to cover all of the overhead when processing data and sending secure consent documents to the writing agent.
Our Consent Form does NOT affect your contracts. We use your information ONLY to set up your unique link.
We use anything your client inputs into our system to provide you with consent ONLY. To protect you, we included this in our Business Associate Agreement. In the BAA, we specifically confirm that we only use the information provided to facilitate the services you have agreed to.
When you first sign up, your link is created in our system but does not go live until we check that the information you put in was done so correctly. While your link wasn't live, clicking on it would bring you to our agency error page. Please allow up to 1 business day to have your PURL go live.
The most cost-effective solution that we have for agencies under 10 agents is to have your team sign up under the individual agent link. Our agency model starts at $200 per month with a larger focus on helping the agency with bulk onboarding downline agents.
While there are other companies that have Snap Health apps, they are not us! At this point in time we do not have a mobile app.
We don't have full customization as all of the brokers who currently have a dynamic link would not necessarily want broad changes affecting their version based on any one individual end user trying to make changes. We will have more features and functionality in the future, but we aren't there yet.
Once signed up, you will receive a unique link. If you open the link on your phone, you will see that the consent form is mobile friendly. All you would have to do is send the link via text and the client would be able to complete consent on their phone as well.
SnapHealth is not a carrier. We are an independent technology company founded by brokers. We are publishing software that we built for our own agency, to help more brokers support their clients.
To an extent. Our links allow for certain variables to be passed in the URL. With this option, you can prefill your clients name, zip code and email. We never suggest attempting to prefill everything, pre click the consent buttons or signing for a client. The goal with consent forms is to be sure your clients have personally given you the necessary authority to complete the enrollment.
